How To Apply for the Mentorship and Fellowship

Completed application forms and all required supplemental information listed below should be postmarked by the application deadline for each program. Print out and complete all components of the application form. You cannot apply online at this time. Send materials in ONE packet to the program.
If you are applying for a scholarship, all scholarship application materials must accompany your admissions application.

Application Components
- Letter of Intent identifying the program(s) for which you are seeking entrance.
- Resume listing continuing education courses, employment, and other recognition. Course work on resume should include name of the course, instructor, location, and approximate number of contact hours.
- Official copies of all college and university transcripts. If the college allows YOU to receive the transcript, please send transcript in the sealed envelope with your application. Please make a list of all university transcripts and the nature of the course work you undertook.
- GRE scores (optional)
- Two Applicant Rating Forms should be INCLUDED with your application.
- Select two individuals who know your academic and clinical potential as a manual therapist.
- Give each individual the attached "Introductory letter for Persons Providing Recommendations" and one copy of the Applicant Rating Form.
- Ask individual to complete the Applicant Rating Form and include comments or a letter of recommendation.
- Have individual place the Applicant Rating Form, and letter of recommendation in a sealed envelope and sign his/her name across the seal.
- Place the sealed envelope with the application which you mail to us.

- Phone Interviews and Personal Interviews
Three-month Mentorship applicants: Applicants who pass the initial screening will be interviewed by telephone. Contact us for the phone interview dates.
Six-month Fellowship applicants: Applicants who pass the initial screening will be asked to come to a personal interview. Applicants to the combined Mentorship/Fellowship will be asked to come for a personal interview. Contact us for the interview dates for the year that you are applying for admission. - California Licensure
Out-of-state applicants are encouraged to initiate their California licensure at the same time that application is made to the program. Participants must show evidence of full licensure six weeks before entering the program. To obtain the California licensure application, contact the Physical Therapy Examining Committee, Tel: 916-561-8200, 1434 Howe Avenue #92, Sacramento, California 95825. When possible, materials should be sent by express mail. The licensure process normally takes 4-5 months to complete for U.S. trained therapists.
Non-U.S. trained applicants must complete an internship in a California hospital before becoming licensed and before applying for admission to the residency. Canadian trained physical therapists who are graduates of APTA approved curricula should contact the Physical Therapy Examining Committee related to licensure requirements. - Application Fee
$25 application fee per program, (i.e., $25 for Mentorship, $25 for Fellowship). Application fee is waived for Kaiser employees. Make check payable to the Physical Therapy Fellowship Program and send to our department address. - All materials must be received by the application deadline. If you miss an application deadline, contact us to determine if there are class slots available due to cancellations.
